| Linkage to Country/Regional Strategy | ADB's commitment to a multi-sector and gender-responsive approach to HIV interventions-embedded in the Strategic Directions Paper-has been further reiterated in the Joint Initiative by Development Agencies for the Infrastructure Sectors to Mitigate the Spread of HIV/AIDS signed by the ADB and other development partners in 2006. Most recently, the long-term strategic framework of the ADB (2008) reiterates the bank's commitment to the mitigation of the HIV risks and impacts in its protfolio by continuing to incorporate activities to control transmission and spread of communicable diseases particularly HIV, in its transport and infrastructure projects. | |||
| Impact | The expected impact of the TA will be an attempt to reduce the incidence of STI/HIV among construction workers, sex workers and migrant workers in the project area and address some of the factors that contribute to their vulnerability. | |||
| Outcome | Its expected outcome will be expanded networks and stronger service delivery capabilities for HIV prevention in five selected North East States (Assam, Meghalaya, Mizoram, Manipur and Nagaland) through public-private partnership. The TA will complement the existing effort of NACO in the North East. | |||
| Outputs and Timeframe |
The TA outputs will include: (i) detailed TA work plan to ensure that the TA activities fit with NACP III; (ii) capacity needs assessments; (iii) behavioral baseline reports, (iv) situation analyses reports on institutional framework; (v) training tools and material; (vi) project impact assessment; and (vii) report on: mitigating the social risks and impacts of road projects in the North-East.
The TA will be implemented over 36 months, commencing in September 2009 with completion date 3 years later in 2012. Consultants inputs will amount to: 39 months for national experts, 9 months for national research institute, 12 months for national training institute; and 30-months/each for service delivery NGOs. |

| Summary of Environmental and Social Issues |
The new 2007 estimates released by the National AIDS Control Organization (NACO), supported by UNAIDS and the World Health Organization (WHO), indicate that national adult HIV prevalence in India is approximately 0.34%, which corresponds to an estimated 2.3 million Indian adults (15-49 years) living with HIV. State specific variations in the profile of the epidemic have been observed: several States in the north-eastern part of the country have shown higher HIV prevalence and multiple routes of HIV transmission with diversity between and within States.
In the North East, the HIV epidemic has clearly progressed beyond the prototype of being driven by injecting drug use, and heterosexual transmission has gained momentum. Most of the HIV-related services were concentrated in the urban settings and the geographical reach into rural areas was poor. Women with unequal access to health and nutrition and lesser level of participation in social and political decision making processes are particularly at a disadvantage, increasing their vulnerability to HIV significantly. Nagaland and Manipur are high-prevalence States in the North East. Mizoram is highly vulnerable and Meghalaya reported very low number of HIV infections and AIDS cases. |

| Stakeholder Participation and Consultation | TA fact-finding missions in May-July 2007 at central, State and district levels and follow-on discussions with the national AIDS Control Organizations (NACO) and the Joint United Nations Programme on HIV/AIDS (UNAIDS) in April 2009 confirmed the relevance of the proposed activities under the TA as well as the Government's commitment to work with ADB in the implementation of the proposed activities. | |||

| Consulting Services |
The National AIDS Council Organization (NACO) will be the Executing Agency for the TA. The NACO Regional Office -through the UNAIDS Secretariat in New Delhi and other partners - will be engaged as implementing agency (IA) to undertake the consulting services under this project, based on direct selection procedure, in accordance with ADB's Guidelines on the Use of Consultants (2007, as amended from time to time).
