Linkage to Country/Regional Strategy Strategy 2020 confirms that health is vital to development, productivity, social inclusion and gender equity. The Strategy also recognizes that health affects¿and is affected by¿poverty and development through many complex pathways, including malnutrition. The strategy envisages ADB working even more closely in partnership with specialized agencies focused on health and other development partners with complementary technical skills. ADB's Special Evaluation Study of Selected ADB Interventions on Nutrition and Food Fortification highlighted the need to strengthen national food fortification as the most cost-effective intervention to reduce micronutrient deficiencies. ADB's Operational Plan for Improving Health Access and Outcomes under Strategy 2020, approved in October 2008, recommends that ADB may be engaged in health sector projects if the activity is highly selective and ADB's engagement is in partnership with relevant technical agencies. The proposed JFPR grant will pilot-test a new approach, in partnership with The Micronutrient Initiative, in a highly selective activity that will also help in achieving the MDGs.
Impact The Project will support MDGs for poverty reduction, gender equality and maternal and child mortality. Reduced anemia improves adult work performance in agriculture and manual labor. Anemia reduction disprportionately benefits women and girls who suffer greater prevalence. Anemia is associated with perinatal mortality, folic acid deficiency with rates of death from birth defects, and vitamin A deficiency with 23% of deaths from diarrhea and respiratory infection. Anemia is also associated with about 20% of maternal mortality worldwide.
Outcome The short-term outcome will be that about 200,000 people in the project area consume about 37,000 MT of flour fortified with iron, folic acid, and vitamin A over a period of two years. The long-term outcome will be a national consensus on plan and financing for sustainable expansion to scale.
Outputs and Timeframe The project will be implemented over a period of 3 years. The project outputs will be (i) 65 Village Development Committees with capacity, resources and community project plan; (ii) 360 millers with capacity to fortify built; (iii) consumers accept and pay for fortified flour; (iv) national, regional and district government offices with capacity to assure quality; and (v) health and economic rationale for small mill fortification built.
Geographical Location About 65 VDCs in one district of Nepal, to be selected at project start-up.

Procurement
  Consulting Services The Micronutrient Initiatve will be recruited by ADB through direct selection to act as the project technical advisor. Local NGOs and consultants will be contracted for social mobilization and capacity building activities by the PIU in accordance with ADB's Guidelines on the Use of Consultants (2007, as amended from time to time).
  Civil Works/Goods All procurement under the grant will be conducted in accordance with ADB's Procurement Guidelines (2007, amended from time to time).
